Our Professional Services custom request forms (Custom Report, Custom Alert, Custom Search, Custom Task, Custom Vital and Custom Documentation) will be relocated to Netsmart Connect on 10/1! You can access Netsmart Connect using the following link: NetsmartCONNECT Sign in (b2clogin.com).

Once logged in, click on Click ‘Support’-> ‘Create case-catalog’-> ‘TheraOffice.’ Lastly, choose the appropriate form based on the type of request you are interested in. Once the form is completed, a case will open, and it will initiate the quoting process on our end.

If you do not have a Netsmart Connect account, please create one by clicking on “Sign Up Now” on the login screen that appears when you click on the link above. If you need assistance, please contact our support team at 866-562-8413 x 2.

Any requests that come in on an old form during the month of October will be honored; however, a representative will call you to walk through the new process so that it can get submitted through Netsmart Connect. This will allow us to be much more transparent with due dates as well as allow internal teams to communicate more efficiently to get your request completed in a timely manner. Please note that on November 1st, we will no longer accept custom requests on the old forms.
